Communicate to Primary Care Physicians through the Coordination of Care Form and/or by telephone each consumer’s level of care, medication/changes, and any changes noted in physical health needs. Document all communication in consumer chart. ? Maintain knowledge of prescription insurance coverage and, as needed, complete prior authorizations for certain medications for insurance companies and pharmacies. ? Maintain relationship with pharmaceutical representatives to continue receiving sample medications. ? Maintain record of all sample medications. Do regular counts to ensure adequate supply and order more when supply runs low. ? Provide sample medications to consumers with standing med orders when necessary. ? Call in short-term supplies of medications to pharmacies when consumer has standing medication order and is due to run out prior to next appointment. ? Link consumers with medication assistance programs as necessary for medications not covered by insurance or for consumers without insurance. Maintain knowledge of assistance programs and continue to build knowledge in this area. ? Educate consumers on their medications including desired effects and possible side effects. ? Give shots to consumers as needed and on schedule. ? Review self-report Health Screens and sign when review complete; if Health Screen notes possible physical health concerns, complete full health assessment with consumer present. ? Link to Primary Care Physician when necessary. ? Assist and keep the assigned Psychiatrist informed of any changes or pertinent information they may need. ? Monitor changes in physical health needs of consumer and coordinate with case manager and primary care physician as needed. ? Maintain credentials and certifications as required by contract agency, and State of Michigan. ? Adhere to recipient rights and confidentiality standards at all times.Listing originally posted at http
